Insurance Information
We accept a wide range of insurance providers.  For the non insured we have a sliding fee.  No one will be denied access to services due to the inability to pay.
New Patient Form
New patients will need to complete the New Patient Information Form. To speed up the process, new patients can print the New Patient Form here.    
Payments
We are committed to providing high quality health care at a reasonable cost. Please do not hesitate to discuss our fees freely if you have questions.

As a convenience to our patients, we will process insurance forms for many companies. We ask that you become familiar with your own coverage so that you are prepared to pay for non-covered services and copays at the time of your visit.

If you are experiencing financial difficulty, and are unable to pay for our service at the time of your visit, regular weekly or monthly payments will be accepted. If you have no insurance, we can discount the fee depending on your income.

Lab & Testing Information
Family Medicine of Warren accepts walk ins for lab draws at our Warren office between 7:30 a.m. and 9:00 a.m. Monday thru Friday.  All others should be scheduled ahead of time.  Most lab work is being processed by Associated Clinical Laboratories.  You will be billed by them directly.
Prayer Partner Requests
“As a Christian practice, we believe in the power of prayer”. Patient can email us with any requests for prayer. Mission Trip Information
With our Mission Statement in mind, our staff and physicians are encouraged to participate in short term mission trips and many have done so over the years. Our staff have been to Haiti, Russia, Africa, Honduras and closer to home in West Virginia and also helping with the cleanup after Hurricane Sandy.
Hospitalist Program
Hospitalists are trained and uniquely qualified to care for our patients admitted to the hospital. The hospitalist has access to your medical record during your stay in the hospital and is in close contact with your family physician.
